Client Bill of Rights

Graceful Nurture Home Health, LLC (“GNHH”) is committed to providing compassionate, respectful, and high-
quality home-care services. Clients receiving services from GNHH have fundamental rights protected under federal and state laws. These rights apply to all individuals served by GNHH across the United States.

Right to Non-Discrimination

You have the right to receive services without discrimination based on:

  • Race, color, or ethnicity
  • National origin or language
  • Limited English Proficiency (LEP)
  • Age
  • Disability (physical, developmental, mental health, or sensory)
  • Religion, faith, or creed
  • Sex, gender, gender identity, or sexual orientation
  • Marital, familial, or parental status
  • Pregnancy
  • Veteran or military status
  • Immigration or citizenship status
  • Any characteristic protected by federal, state, or local law

GNHH will provide legally required accommodations and interpreter services at no cost to you.

Right to Respect, Dignity, and Privacy

You have the right to:

  • Be treated with courtesy, respect, and dignity
  • Receive care in a safe, comfortable, and private environment
  • Have your cultural, spiritual, and personal values respected
  • Expect staff to protect your privacy during all services
  • Participate in decisions regarding your care and services

Right to Information and Participation

You have the right to:

  • Receive clear explanations about your care, services, and expectations
  • Participate actively in creating and updating your service plan
  • Be informed promptly of any changes in services or scheduling
  • Ask questions and receive understandable answers
  • Receive information in your preferred language (with interpreter support when necessary)

Right to Quality Care

You have the right to:

  • Competent, trained, and supervised caregivers
  • Services delivered according to professional standards
  • Consistent and timely care
  • Care that supports your health, safety, comfort, and independence

Right to Choose Providers and Services

You have the right to:

  • Choose or decline GNHH services
  • Request a change of caregiver or staff member
  • Seek care from another agency or provider at any time
  • Receive information about alternative resources or service options

Right to Confidentiality and HIPAA Protection

You have the right to:

  • Confidential handling of your personal and health information
  • Receive GNHH’s HIPAA Notice of Privacy Practices
  • Approve or deny the release of your records, except when disclosure is required by law
  • Request copies of your records

Right to Safety and Protection

You have the right to:

  • Receive services in a safe and clean environment
  • Be free from abuse, neglect, exploitation, retaliation, or harassment
  • Report safety concerns without fear of reprisal
  • Receive care from staff who are properly screened, trained, and supervised

If you ever feel unsafe, you may report your concerns immediately.

Right to Voice Complaints or Grievances

You have the right to:

  • File a complaint without fear of discrimination or retaliation
  • Report concerns verbally or in writing
  • Receive a timely response and fair review
  • Involve a family member, representative, or advocate
  • File complaints internally or with external agencies

Right to Informed Consent and Refusal

You have the right to:

  • Receive explanations before services are provided
  • Consent to or refuse any service
  • Withdraw consent at any time
  • Be informed of potential consequences of refusal

Right to Financial Information and Transparency

You have the right to:

  • Clear information about fees, rates, and billing practices
  • Notification of services covered by insurance or other payors
  • Written notice of any changes in charges
  • Assurance that you will not be billed for services you did not receive

Right to Access Your Records

You have the right to:

  • Review your records within a reasonable timeframe
  • Request corrections to inaccurate information
  • Authorize a representative to access your records, if desired

Right to Decide About Advance Directives

You have the right to:

  • Create or update advance directives
  • Appoint a healthcare proxy or representative
  • Have your wishes honored within the limits of the law
  • Receive care regardless of whether you have advance directives on file

Right to Personal Freedom and Independence

You have the right to:

  • Make personal decisions about your daily activities
  • Participate in community or social activities as desired
  • Maintain independence consistent with safety
  • Be free from unnecessary restraints or restrictions

Right to Professional Boundaries and Ethical Care

You have the right to:

  • Expect staff to maintain professional boundaries
  • Protection from conflicts of interest
  • Care that adheres to ethical, legal, and professional standards

Right to Notification of Rights

GNHH will ensure that:

  • You receive this Client Bill of Rights at admission
  • Rights are explained in a way you understand
  • Updated versions are provided if changes occur
  • State-specific rights (when applicable) are posted at GNHH home offices
